For women with particularly small breasts, the augmentation mammoplasty is a procedure designed to enhance and improve the appearance of the breasts. During the augmentation mammoplasty, the surgeon inserts an implant with a silicone shell filled with saline (salt water) behind the breast or behind the muscle of the chest wall through a small incision. I use a small incision at the lower border of the areola which leaves an inconspicuous scar and I almost always insert the implant behind the muscle of the chest wall. This gives a more natural appearance, keeps the implant soft and most importantly makes it easier for the radiologist to read a mammogram.
Often the candidates for this procedure are women who have experienced breast shrinkage following pregnancies. The procedure can restore fullness and reduce sagging. Additionally, it is often the treatment of choice for women who have been naturally endowed with small breasts.
During the initial consultation, the patient and I together decide what size implant will look best. The size of the implant is limited to the degree of stretch the existing breast can tolerate. Our goal is to create a natural looking breast that is proportional to the size of the individual.
As is the case with any surgical procedure, there are risks associated with augmentation. Sometimes, the breasts may become somewhat more firm -- a fact attributable to the tightening of a capsule which forms around the implant. Today, the occurrence of firmness is very low. Additionally, a small number of patients note some loss of sensation in the nipple area. Also, one must realize the implants are a man-made device and can leak, requiring replacement.
The augmentation mammoplasty is performed under general anesthesia. The patient may usually return to daily activities within one week following surgery while avoiding strenuous activities for three weeks.
Breast augmentation remains one of the most popular cosmetic procedures. Typically, patients' expectations of the surgery are met, and they are very pleased with their new silhouettes.
